TRANSITION MEETING A transition meeting is held approximately 10 days prior to the client's last day in the program. Those attending the meeting the client, AOD Coordinator, Mental Health, Primary Counselor, family members, Probation Officer, and aftercare provider. agenda consists of a verbal review of program participation, comprehensive assessment contents review, copy of DSMIV diagnosis given, and executing transition plan/WNRYC recommendations and aftercare plan.
