Levels of “In-House” Aftercare Treatment Services

Our mission is to provide continuity of AOD care including services after client discharge.  In addition to community-based support for the probation aftercare plan, WNRYC offers four different levels of “in-house” aftercare that are seen as a continuation of the overall program.  These levels of aftercare will be provided through a transition meeting agreement with the following treatment team members:  WNRYC staff, the probation officer, family members, the client and community providers.   

Level 1.  TEACHING / MENTORING

·        CRITERIA
A visit of less than a day
The client is a positive peer and is able to give constructive feedback to the community.

·        GOALS
The client gives a presentation or teaches a group.
Reaffirmation of sobriety

Level 2.  TRIGGER MANAGEMENT

·        CRITERIA
A one to three day stay, usually a weekend or so.
The client is a positive peer needing a refresher.
The client may need structure and support during a trigger period.

·        GOALS
To work through any trigger issues and adapt new insights into aftercare and treatment plan.
  

Level 3.  RELAPSE INTERVENTION

·        CRITERIA
A ten to fourteen day stay for a physical relapse.
The client has begun using drugs/alcohol again, but support systems are still intact.  Client is highly motivated.

·        GOALS
To work through relapse and identify, step by step, what led to relapse.
Create and begin an improved aftercare plan.  
 

Level 4.  RE-INITIATE TREATMENT

·        CRITERIA
A 70-day stay, re-starting treatment.
The client has relapsed, begun using drugs and alcohol again, and less structured services are not useful.
Relapse Intervention has failed

·        GOALS
To reinitiate full treatment/program and identify what worked and what led to relapse.
